บทคัดย่อ

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษารูปแบบการจัดการดูแล ปัญหาความต้องการ และเสนอแนะแนวทางในการพัฒนารูปแบบการจัดการดูแลระยะยาวกลุ่มผู้สูงอายุติดเตียง เน้นการวิจัยเชิงคุณภาพโดยการสัมภาษณ์เชิงลึกผู้สูงอายุติดเตียงหรือผู้ดูแล และสนทนากลุ่มองค์กรต่างๆ ในตำบลท่าไม้ อำเภอท่ามะกา และตำบลจรเข้เผือก อำเภอด่านมะขามเตี้ย จังหวัดกาญจนบุรี ซึ่งจะนำข้อมูลที่ได้มาวิเคราะห์เชิงเนื้อหา สรุปผล และนำเสนอตามประเด็นต่างๆ ที่กำหนดไว้

ผลการวิจัยพบว่า รูปแบบการจัดการดูแลผู้สูงอายุติดเตียง แบ่งออกเป็น 8 รูปแบบ คือ การดูแลตนเอง การดูแลโดยครอบครัว การดูแลโดยเพื่อนบ้าน การดูแลโดยผู้นำชุมชน การดูแลโดยชมรมผู้สูงอายุ การดูแลโดยอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) ในกรณีที่ไม่มีผู้ดูแล มีผู้ดูแล และมีผู้ดูแลแต่ไม่ยอมดูแล การดูแลโดยองค์กรปกครองส่วนท้องถิ่น (อปท.) และการดูแลโดยสถานบริการสุขภาพ ปัญหาอุปสรรคที่พบคือ ผู้สูงอายุติดเตียงไม่ให้ความร่วมมือในการทำกายภาพบำบัด ผู้ดูแลไม่มีเวลาและรู้สึกเหนื่อย/ท้อแท้ ขาดนักกายภาพบำบัด/นักพัฒนาสังคมลงพื้นที่ช่วยเหลือดูแล และขาดวัสดุอุปกรณ์ที่จำเป็นต่างๆ ดังนั้นจึงมีความต้องการอบรมให้ความรู้แก่ผู้ดูแล/อสม. เรื่องการรักษาพยาบาล/ทำกายภาพบำบัด จัดตั้งศูนย์ฟื้นฟูและดูแลผู้พิการในชุมชน และให้องค์กรต่างๆ ในชุมชนลงพื้นที่ร่วมกัน เพื่อช่วยเหลือดูแลสุขภาพ มอบอาหาร ยา เงิน และวัสดุอุปกรณ์ที่จำเป็นต่างๆ

ข้อเสนอแนะคือ สร้างการมีส่วนร่วมของชุมชน เน้นการดูแลจากครอบครัวเป็นหลักโดยต้องอบรมให้ความรู้ในการดูแล ทำกายภาพบำบัด ร่วมกับให้ อสม. เยี่ยมเยียน ให้กำลังใจ ดูแลสุขภาพ ประสานการส่งต่อกรณีฉุกเฉิน ส่วนหน่วยงานภาครัฐทำหน้าที่สนับสนุนบุคลากร งบประมาณ และวัสดุอุปกรณ์ที่จำเป็นต่างๆ ตลอดจนกำหนดเป็นนโยบายการดูแลระยะยาวที่ชัดเจน เพื่อเป็นแนวทางปฏิบัติเดียวกันทั้งประเทศ

Abstract

The objective of this research was to study the model of need, problem and care management and recommendation in the model of long term care for the bed bound elder management development. It was emphasized as a qualitative research by using in-depth interview the bed bound elders or care givers and some organizations in Tha Mai sub-district, Tha Ma Ka District and Cho Ra Khe Phuak sub-district, Dan Ma Kham Tia District in Khanchanaburi Province. Data was essential analyzed, concluded and presented according to the itemized provisions.

The result found that the model of long term care for the bed bound elder was divided into 8 aspects. Themselves care, neighbors, community leaders, elderly clubs, village health volunteers (in case of there was no care givers or neglected by care givers), local government organization and health service centers. The problem and obstacle were found that the bed bound elder did not participate in physical therapy, the care givers did not have enough time and felt tired, there was no physical therapists/community development staff in that area did not help them and lack of essential physical equipments therefore providing a training course for care givers/village health volunteers about treatment and physical therapy, day care center in community is needed. Some organizations in each communities coordinated together in order to help their health, food, medicines, money and some necessary materials and equipments.

The recommendation was community participation building to aim by their families with a training and knowledge in caring, physical therapy and cooperat with village health volunteers to visit, encourage and give health care and coordinate with referral system in case of emergency. For the governmen organizations supported for manpower, budget and essential equipment including a clear long term policy provision in care management throughout the whole country for the same implementation.
